AIS Home Visit Program

The Advanced Illness Service (AIS) program is a collaboration between Highmark and Aspire Health. Together, we arrange home visits from a nurse practitioner to help members manage serious illnesses – free of charge.

What to expect from your free AIS home visit

Aspire nurse practitioners work with a team of doctors, nurses, and social workers to help you:

— Manage your symptoms: Our team helps you manage symptoms such as pain, shortness of breath, fatigue, constipation, nausea, loss of appetite, difficulty sleeping, and depression.

— Work towards your goals: We will work with you and your family to help ensure your care plan aligns with your health care wishes.

— Coordinate your care: Our team works closely with your doctors and health care providers. We can help identify additional resources that may be beneficial to you.

— Educate you and your loved ones: We will make sure you, and your family, understand your illness, care plan, and treatments. We want you to feel confident in your health care decision-making and your future plans.

This program is available to all Highmark Medicare members, but you are under no obligation to participate.
